Care Transitions

Amedisys has the industry’s first-ever nationwide Care Transitions program. Amedisys Care Transitions is designed to reduce unnecessary hospital readmissions through patient and caregiver health coaching and care coordination, which starts in the hospital and continues throughout completion of the patient’s home health plan of care.

Chronic Care Management

Our chronic care programs were designed to empower our patients so they can experience an optimal quality of life. We use the most current medical evidence to develop the clinical procedures and patient education for each of Amedisys' chronic care programs, including: heart conditions, diabetes, wound care, behavioral health, rehab, orthopedics, dementia, COPD, and kidney disease.

Mercury Doc

Our web-based patient management portal for physicians enables real-time communication and collaboration between you and our clinicians regarding your patient’s plan of care. MercuryDoc can improve the efficiency of your practice by:

  • providing secure access to your patients’ records, including graphed trends and vital signs
  • helping you manage Plans of Care (485s), supplemental orders and patient referrals
  • tracking and reporting time spent on Care Plan Oversight.

Point-of-Care

Our point-of-care laptops empower our clinicians to document and access patient information and our evidence-based clinical protocols for every patient visit. This technology, paired with Mercury Doc, can provide near real-time updates that help coordinate high-quality home health care.

Telehealth

Telehealth monitoring increases the frequency and consistency of measurements of a patient's condition, enhancing the health care providers' ability to recognize early warning signs and make rapid decisions about the patients' plan of care. Amedisys currently offers telehealth monitoring at more than 200 locations across the nation.
