Patient Eligibility

Our highly skilled nurses, therapists and aides will work closely with your practice to coordinate a continuum of care for your patients. The sooner you engage us in the support of your patients regarding their new medication, diagnosis or condition, the more effective our clinical programs and your patients’ outcomes can be. There is a better transitional impact early on that can help prevent hospitalization for your patients.

Checklist for Patient Eligibility:

As a health care provider, you may know of patients who:

  • Have newly diagnosed conditions with new or revised medications requiring additional education and monitoring
  • Are habitual callers into your office with questions about medications, symptoms they are experiencing, etc.
  • Need follow-up care after hospitalization or medication teaching and/or management, but due to homebound status, have trouble getting to your office for care
  • Require teaching on a disease process or medications
  • Have been recently hospitalized for a newly diagnosed condition or exacerbation of their disease process
